The applicants need to submit a photocopy of their Aadhaar card along with the application form. The application form can be submitted at Mo Seba Kendras or Jana Seba Kendras,” Parida added.
The applicants need not have to pay any fee for the offline application form and submission, the Deputy CM said, adding that, in case of any doubts, applicants call the Subhadra toll-free number 14678 for clarification.
Here’s the Application Procedure:
- All eligible women would be required to apply for availing benefits under the scheme.
- The applications may be made offline through a printed form, as well as online through the SUBHADRA portal.
- Sufficient pre-printed application forms will be made available free of cost at different locations such as Anganwadi Centres, Block Offices, Urban Local Body Offices, Mo Seba Kendras, Common Service Centres, etc.
- The applicant will have to fill out the form and submit the same at the nearest Mo Seba Kendra / Common Service Centre.
- In case of any discrepancy in the form submitted by the applicant and the Aadhaar, the information as present in the Aadhaar would be taken as final.
