According to an official, the incident happened at around 3.30 pm in the 8-coach DEMU train between Ahmednagar and Narayanpur stations.
The firefighter teams are yet to establish the exact reason behind the fire mishap.
Though the exact reason behind the fire is not clear, all the patients in the ward as well as in the ICU were subsequently evacuated to safety.
Bhopal railway department said a technical team has reached the spot and is monitoring the situation.
Passengers reportedly pulled chain after detecting the smoke
The fire mishap caused panic as the surrounding area was completely engulfed in thick, billowing smoke, from the burning medicines and other materials, intensifying the impact of the inferno.
As it was their first priority to rescue the trapped persons, the firefighters took the trapped persons out of the spa to another building through a ladder.

As per initial reports, the Mo Bus caught fire near TCI Square due to some technical glitch. After noticing the smoke, all the passengers got down from the bus. A major tragedy was averted as all the passengers got down from the bus which was completely gutted within minutes.
Though there has been no loss of life, several properties inside the storeroom have been gutted in the fire mishap.
The firecrackers were manufactured illegally in the quarters, sources informed, adding that the quarters was alloted to one Niranjan Lenka, who is posted at State secretariat.
Thatched houses and haystacks catching fire on their own have left even the firefighters in a dilemma.
Meanwhile, shop owners and locals expressed outrage over the engagement of only one fire brigade team to douse the massive blaze.
The mishap occurred at one Sheikh Bhallu’s house. The cylinder caught fire when his wife was cooking food in the evening.
According to a witness, the fire started at around 7.30 pm in the complex that houses several offices.
